Disciple Maker Index (DMI) Survey

Please help us by completing a 10-15 minute survey by completing it online or by a paper copy.  The survey will only be available from April 4 through April 18th.  We ask you to reflect on your own spiritual growth and provide feedback on our parish’s efforts to help you grow.  All responses will be confidential and the parish will only receive information about the community as a whole.

Paper Survey:  Copies can be found on the table in the church entrance.  When completed, please drop it off in the basket by the paper copies on the table.  Thank you.
